Churubusco High School Athletic physicals is 8:30 to 9:00 AM on Saturday April 11th at Fort Wayne Orthopedics office (7601 W. Jefferson, Fort Wayne). Please stop in the main office and pick up the forms you need BEFORE you go.  Please have them filled out prior to arriving at FWO to speed up the process.  See below for more information.

The Orthopedic Hospital and Fort Wayne Orthopedics are pleased to daily serve your high school athletes with  sports medicine services. One of the many services we annually provide is  free pre-season athletic physicals.  This year we will again offer pre-season athletic physicals on Saturday April 11,  2015.  Please encourage your athletes to attend in the recommended half hour slots so we have a smooth and efficient flow of the anticipated 800 to 1000 athletes. We will be staffing the athletic physicals with dozens of Physicians, Physician Assistants, Nurse Practitioners and we will also have a cardiologist in house to consult if needed. Please see the attached schedule to confirm the suggested time for your athletes. The physicals are for high school athletes and those graduating into high school. As always if an athlete has a competition or conflict you are welcome to advise the individual athlete to arrive anytime between 8:00 AM and 10:30 AM. Since we will have nearly 100 of our TOH and FWO personal involved in providing a satisfying experience,  we cannot have additional times nor dates for those who cannot attend. We also remind every student athlete attending, verbally and in writing, that we do not keep copies as they are responsible for delivering the athletic physical forms to their Athletic Director. We anticipate that the students will arrive with athletic physical forms in hand ( we will have copies for those who forget) with the medical history portion completed by parents since the medical history is a critical component of the athletic physical.
